THE OPERATIONAL ADVANTAGE
We built xmplr around PDN: care delivered shift after shift, over the whole length of a patient’s care. The care plan is the core. The shift note, the medical record, and the workflows around them exist to carry it out. Our goal is to connect the rest of the business to that plan, from staffing and supervision to reimbursement.

Bring skills, distance, workload, and familiarity with the patient into the staffing decision. The scheduler can see the fit and decide.
Scheduling previewThe work continues after the note. Care plans and clinical records support insurance submission; automating claim generation and submission is the next phase.
WHY WE BUILT XMPLR
Every shift includes care, documentation, and the responsibility to get both right. We built xmplr so that every nurse on a patient works from the same plan and charts against the same structure, instead of each one documenting it their own way.
A short animated story · 36 secondsTHE PLATFORM / BUILT AROUND PDN
The note is a mobile app the nurse works in during the shift, a tap at a time, not written up when it is over. The supervisor gets a clear record from it. The rest of the agency’s work connects to the same place.
01 / CARE
The supervisor establishes the patient’s baseline, required care, and timing. That evaluation shapes the guided note and the CMS-485. Claims automation is the next phase.

COMPLIANCE & READINESS PLANNED
Credentials expire. Authorizations need renewal. Signatures need follow-up. We plan to bring those requirements together so the office can see what is due and act on it.
FAMILIES PLANNED
In pediatric care, parents are involved every day. We want them to see who is coming, know their care team, and find their education and training records.
Availability varies by capability. Planned features reflect product direction; integration and rollout requirements vary by agency.
01 / THE CLINICAL CORE
Define the care in the evaluation.
Carry it into the nurse’s note.
The supervising nurse records observations, establishes normal limits, and defines the required care and its timing. It is a detailed assessment. That work gives the bedside nurse patient-specific guidance throughout the shift.

Guided by the patient’s care plan.
A shared structure for charting, with normal limits and scheduled care tailored to each patient.
Structured transfer of care, with geolocation verification to document where the handoff occurs.
Evaluations, guided notes, medication administration records, and CMS-485 generation in one platform.
FROM CARE TO REIMBURSEMENT
The care is documented as the shift happens, and the CMS-485 is generated from the evaluation. Carrying that documentation through to submission and automated claims is the next phase.

03 / BUILT TO ADAPT
We started with nursing and software experience, and a goal: build the software a private duty nursing agency needs to operate. The clinical foundation is working. We’re developing the wider product around it.
PURPOSE-BUILT. ADAPTABLE.
Every agency has its own priorities. We want a shared product that can be tailored around them. Patient-specific care is built in; configurable staffing is taking shape in the scheduling beta.
The roadmap includes oversight across multiple agencies, with shared standards and room for local workflows. That is part of xmplr’s product direction.
TECHNOLOGY & PRODUCT FOUNDATION
The value is not a list of frameworks. It is a custom foundation that connects clinical work, agency operations, and the business around them, with room to keep expanding.
RESPONSIBLE ACCESS
Role-based access and dashboards keep nursing, supervision, and office work organized around each person’s responsibilities.
ONE RESPONSIVE PRODUCT
A phone for the shift note. A tablet for the evaluation. A desktop for scheduling and oversight. One web application, shaped around the task.
CONNECTED BY DESIGN
The evaluation shapes the note. The note becomes the record. The record supports the path to reimbursement. Each step carries the work forward.
PURPOSE-BUILT SOFTWARE
xmplr is not a reskinned generic EMR. The application was built around private duty nursing and updated so the product can keep moving forward.
BUILT TO EVOLVE
The clinical core is working and scheduling is taking shape. New capabilities can extend the shared platform instead of becoming another disconnected tool.
A DISCIPLINED ROADMAP
Broader operations and multi-agency oversight are part of the roadmap. Each rollout can be scoped and validated around real agency requirements.
THE ROAD AHEAD
A working clinical core.
A focused product roadmap.
Patient-specific evaluations, guided shift notes, medication records, and generated CMS-485 documents.
Smart nurse matching and scheduling that bring staffing decisions closer to care.
Next-phase claim generation and submission automation, alongside authorizations, EVV, payroll, family connections, and multi-agency oversight.
Discuss the roadmap✨AI WHERE IT FITS
Structured, patient-specific documentation is what makes automated review possible. As that record grows, we plan to use it to surface what falls outside a patient’s established limits and bring the right items to a supervisor sooner. Review stays with your clinicians.
Product direction, not delivery dates. Feature availability and rollout requirements are confirmed before implementation.
